CONNECT-HTN-DM is a community-driven chronic disease initiative focused on improving prevention, early detection, and long-term control of hypertension and diabetes in the DMV areas. Through co-design and participatory approaches, the program expands access to primary care, enhances self-management, and reduces avoidable complications through strengthened community–clinical linkages. Grounded in implementation science and practice, the CONNECT-HTM-DM project generates actionable data to close the policy-to-practice gap and build sustainable, scalable models of care.
The RDN initiative develops a structured, evidence-informed pathway to identify and support patients with treatment-resistant hypertension who may benefit from renal denervation therapy (RDN). Integrated within community and primary care systems, the program strengthens referral networks, patient selection processes, and long-term monitoring to ensure equitable access to advanced hypertension treatment. By aligning clinical innovation with community engagement, ClinRé advances both improved blood pressure control and system-level transformation.
